DOCUMENTS REQUIRED
1. 2 National visa application forms, duly filled out, signed by the person in who’s name
the will be issued, and 2 recent passport size photographs in colour and with white
background attached to the form.
2. Power of attorney for representatives in case the application is not presented by the
applicant.
3. Payment of Visa fees must be done only through demand draft in favour of
CONSULATE GENERAL OF SPAIN. Two photocopies of demand draft must be
submitted.
4. Covering letter explaining the purpose of the visa application, either in Spanish or if
written in any other language translated to Spanish. A photocopy of the same must
be submitted.
5. Form EX-01 – Application form for authorisation of Residence. A photocopy of the
same must be submitted. Consult corresponding fee.
6. Form Modelo 790, code 052 - Proof of payment of the fees for the residence
application procedures. A photocopy of the same must be submitted.
7. Passport with a minimum validity of 1 year and photocopy of all pages of the same.
8. Public or private medical insurance policy issued by an insurance company
authorised to operate in Spain. The medical insurance policy must cover the entire
period of stay in Spain (or at least one year), the medical expenses and repatriation
associated with an accident or sudden illness. A photocopy of the same must be
submitted
9. Police Clearance Certificate issued by the competent authorities of the country
where the applicant has resided in the last 5 years prior to the submission of the
visa application, legalised + authenticated/ apostilled. Only in case of adults over
the penal age.
10. Medical certificate issued by any registered medical practitioner updated.
11. Proof of sufficient liquid economic means to support the living and stay expenses,
including, the expenses of the family for the proposed duration of stay and without
undertaking any labour or professional activity.
For principal applicant – 400% of IPREM
For each dependent – 100% of IPREM
12. For Dependents of Principal Applicant - Original document that justifies the family
bond relationship with the family members of the applicant. Certificates such as
birth/marriage should be apostilled or legalized. A photocopy of the same must be
submitted.
13. For Minors dependent on the principal applicant - Documents of school admission in
Spain

OTHER REQUIREMENTS AND OBSERVATIONS
TRANSLATION: Sworn translation to Spanish from source language if the original
documents are in a regional/foreign language (except passports and economic proof
documents).

PHOTOCOPIES: Kindly note that for each original document presented a photocopy of
the same must be submitted. The photocopies must be made after legalising the
translations and/or the documents.
INTERVIEWS: At any time during the visa application procedure, this Consulate General
may require the personal appearance of the applicant, and if considered necessary,
conduct a personal interview.
THIS CONSULATE GENERAL MAY REQUEST FOR ANY ADDITIONAL DOCUMENTATION.
